For your application to be considered you will need to follow the instructions
below:
| 1. | Submit one copy of the application and supporting materials to the Valley of Dayton by April 1 |
| 2. | If you have more than one grandfather who is a Scottish Rite Mason submit both names. You may also submit name of great grandfather if applicable. |
| 3. | In answer to item 4c on the application (extracurricular activities) attach a separate sheet of paper listing those activities and the years you participated in them (freshman, sophomore, junior, senior). |
| 4. | Submit an official and complete copy of your academic record up to the semester or Quarter completed prior to April 1 st of the year you are applying for the scholarship. |
| 5. | Submit results of either your SAT or ACT scores. |
| 6. | Submit a letter of recommendation from a teacher, counselor or advisor. |
